Mechanism of Action:

Pregabalin: Pregabalin binds with high affinity to the alpha2-delta site (an auxiliary subunit of voltage-gated calcium channels) in central nervous system tissues. In animal models of nerve damage, pregabalin has been shown to reduce calcium-dependent release of pro-nociceptive neurotransmitters i n the spinal cord, possibly by disrupting alpha2-delta containing-calcium channel trafficking and/or reducing calcium currents. Evidence from other animal models of nerve damage and persistent pain suggest the anti-nociceptive activities of pregabalin may also b e mediated through interactions with descending noradrenergic and serotonergic pathways originating from the brainstem that modulate pain transmission in the spinal cord.

Nortriptyline: Nortriptyline, a tricyclic antidepressant, inhibits the reuptake of norepinephrine and serotonin, increasing their synaptic concentrations and modulating pain pathways. It is the active metabolite of amitriptyline and enhances analgesic effects by prolonging the action of these neurotransmitters.

Indication:

  • Neuropathic pain management, including pain caused by nerve damage or neuropathy.
  • Diabetic peripheral neuropathy, a common cause of chronic nerve pain in diabetes patients.
  • Postherpetic neuralgia, pain following shingles infection.
  • Sciatic nerve pain and radiculopathy.
  • Fibromyalgia, to help reduce widespread muscle pain and discomfort.
  • Chronic neuropathic pain conditions unresponsive to single-agent therapy.
  • May also be used adjunctively for certain epilepsy cases and generalized anxiety disorder, though primary use is neuropathic pain relief.

This combination is effective due to Pregabalin’s modulation of calcium channels reducing nerve excitability, and Nortriptyline’s inhibition of serotonin and norepinephrine reuptake increasing pain signal inhibition at the CNS level. It is well-tolerated and commonly prescribed for these indications
